Dr. Thomas M. Paumier, DDS, brings a wealth of expertise and dedication to continuing education for dental professionals. A cum laude graduate of The Ohio State University College of Dentistry, Dr. Paumier holds a Bachelor of Science in Microbiology and completed a General Practice Residency (GPR) at St. Elizabeth Medical Center. Since 1988, he has provided outstanding care in private practice in Canton, Ohio, and has been a dedicated educator on the faculty of the Cleveland Clinic Mercy Hospital GPR. As Chairman of the Cleveland Clinic Mercy Hospital Board, Dr. Paumier’s influence extends deeply into clinical education and leadership.

Dr. Paumier has been instrumental in establishing clinical standards that shape antibiotic practices in dentistry. He served on the expert panels for the ADA and the American Academy of Orthopedic Surgeons, co-authoring critical Clinical Practice Guidelines for Antibiotic Prophylaxis for Prosthetic Joint Patients. His role as co-author of the ADA’s Clinical Practice Guideline for Antibiotic Use in Endodontic Infections underscores his commitment to safe, evidence-based patient care. Honored with the ADA’s 2019 Evidence-Based Dentistry Clinical Practice Award, Dr. Paumier continues to drive forward-thinking practices in dentistry.

 SEMINAR TOPICS

1) Antibiotic prophylaxis for prosthetic joint, cardiac and other medical conditions (2hr)

The topic of antibiotic prophylaxis, particularly related to patients with prosthetic joints has been controversial with multiple changes and recommendations over the last 20 years. Are you confused about when it is necessary to use antibiotic prophylaxis for patients with prosthetic joints, cardiac valve replacements, kidney dialysis, AV shunts, transplant patients and other medical conditions?  A review of the risks of antibiotic use including C. diff infections and antimicrobial resistance will be covered. This course will make you confident you are treating these patients appropriately. At the completion of the course you will:

Understand oral bacteremia and the association with distant site infections.
Understand how antibiotic prophylaxis affects oral bacteremia and distant site infections.
Understand the biology and physiology of prosthetic joint infections and infective endocarditis.
Understand the current Clinical Practice Guidelines for antibiotic prophylaxis.
Understand the risks and benefits of antibiotic prophylaxis.

 2) Antibiotic Stewardship and Appropriate Antibiotic Use (1-2hr)

 Antibiotics have improved health outcomes since the discovery of penicillin and its widespread use during WW II. However, their overuse and misuse has now threatened their effectiveness and led to superbug infections that are untreatable. This course will provide antibiotic stewardship guidance and cover the risks of inappropriate antibiotic use, including C. diff and antibiotic resistance. It will also review the latest ADA Clinical Practice Guidelines related to when antibiotics are appropriate for patients with endodontic infections as well as best practices for antibiotic use for extractions, minor oral surgery, implant placement, bone grafting and periodontal treatment.  Prescribing recommendations with sample prescriptions as well as when and why antibiotics are indicated or not indicated will be covered.

 3).  The Oral Health Systemic Health Connection: What’s the Evidence?​(2-4hr)

Nearly every week the mainstream media has articles which discuss the connection between oral health and overall health. Patients and dentists alike are confused about what is fact and what is speculation. A review of evidence-based methodology and how to critically evaluate the literature will be discussed. This course will present the latest evidence supported by credible scientific studies to help dentists understandhow oral health impacts general health and how systemic medical conditions impact oral health.   An emphasis will be placed on how to integrate this information into your practice and communicate the importance of oral health to overall health with patients.  An overview of the interrelationship betweenperiodontal disease and COVID-19, diabetes, cardiovascular health, stroke, cancer, pregnancy, respiratory diseases, rheumatoid arthritis, and Alzheimers disease will be discussed.

 

4) Mastering Composite Restorations (1-4hr)

When faced with fractured or broken-down teeth or dentition, often the choices are limited to crowns or extraction and replacement with implants, fixed bridges or removable partial dentures. Limited finances, an aging population and the improvement of direct esthetic bonded composite expand treatment options. During this course, you will learn how to do routine Class 2 posterior composites, direct composite veneers, post and cores with the core serving as a direct interim crown, direct bonded bridges, and salvaging severely fractured teeth with composite. Complex onlays can often replace crowns when a patient fractures one or more cusps. These techniques will help you solve challenges which you will encounter on a daily basis in your clinical practice. Clinical cases will demonstrate the versatility of well-done composite restorations, giving the esthetics and durability of porcelain with the advantage of simple repair and lower cost.

 

5)How to make Restoration of Dental Implants Affordable and Simple (2hr)

Whether restoring a single missing tooth, a quadrant or an entire arch treatment planning and restoringedentulous spaces with implants can be a challenge. A review of clinical cases from impression to delivery will be covered. Predictable ways to control tissue, simplify provisonalization, take impressions and deliverabutments and crowns will be delivered. Excellent esthetics and function can be obtained in most cases with 2 / 30-60 minute appointments and be fun and profitable.

  
6)   Pain management in Dentistry:  Responsible Opioid Prescribing (2hr)

 This course will cover the types and physiology of pain as well as the goals of pain management.  Dentistry’s role in the opioid crisis will be explained and efforts to minimize opioid use for acute dental pain management by using acetaminophen, NSAIDs, corticosteroids, diphenhydramine, long-acting local anesthetics, ice/heat and muscle relaxants will be covered.  The most recent ADA clinical practice guidelines and scientific research related to acute dental pain management is explained.  When opioids are necessary for pain control a review of morphine milligram equivalents to guide responsible prescribing will be covered and the use of antibiotics for managing toothaches is detailed.  Sample  prescriptions and patient instruction will be provided.
